“A Smile is the Gift that Will Keep Giving for a Lifetime.”
1. Affinity Smiles Dental is giving the gift of a beautiful smile this holiday season to one lucky person completely for FREE (up to $10,000 value).
2. Eligibility. The Contest is open to individuals 18 years or older at the time of entry and is only open to legal residents of Roanoke, Texas and the neighboring cities. Employees of Affinity Smiles Dental and the immediate family members are not eligible.
3. How to apply: Candidates can fill out the application on AffinitySmiles.com or pick up a printed application at Affinity Smiles Dental location at 1112 N. Highway 377, Ste 106, Roanoke, Texas 76262 or Roanoke City Hall at 500 S Oak St, Roanoke, TX 76262. Make sure to fill out the application completely and include a picture of the candidate’s smile and a description of why the candidate deserves to win this Smile Makeover gift. We love to hear your story.
4. Entry. The application will start on November 1, 2020, and end on December 15, 2020. You may submit an application for yourself or on behalf someone that you nominated. 5. Winner Notification and Acceptance. The finalists will be notified by phone, email, and/or text. Each finalist will be required to show up for a screening visit and examination completed by one of the dentists at Affinity Smiles Dental, to determine eligibility and suitability for treatment. The initial screening and examination appointment must be completed within one week upon notification as a finalist. If a potential winner does not respond to the notification within four (4) days, such potential winner will be disqualified. The winner will be announced by December 22, 2021.
